- Medication Disbursment Release Form (if necessary)
- $100.00 deposit (per family), check payable to the Minnesota Foundation for Children

Sample Schedule of Activities
Infants and Toddlers: Directed by licensed day care provider will offer age appropriate activities and large muscle play throughout the adult's daytime activities.
Children & Teens (6-16): Tie-dying, skit planning, scrap booking, scavenger hunt, free play time, sleigh rides, warm cookies, games, talk to the doc, outdoor activities.
Young Adults (17-25): Guitar Hero, Dance Revolution, black light dodgeball, campfire, sleigh rides, massages, pedicures, iTunes creation "Songs of my Heart" cd.
Adults: Massages, scrap booking, sleigh rides, trail hikes, wood working, garden stones, resources table, candlelight dinner, chapel, family photograph.
Space is limited and registrations are accepted in the order they are received. Apply early to ensure availability!
